Background Check Authorization – System Offices Must be completed by the finalist for a position before a formal offer of employment is extended. (HR 32 6/22)

Verification of Degree(s) and/or Licensure Must be completed by the finalist for a position before a formal offer of employment is extended. (HR 41 11/17)

Minor’s Employment Release  Must be completed for employees younger than age 18 and must be retained for duration of employment. (HR 200 12/20)

Statement of Selective Service Registration Status Must be completed by male finalists for positions before a formal offer of employment is extended. (HR 203 10/01)

Forms for managers

Request for Personnel Action – System Offices To be completed by the department head if creating a new position, filling a vacant position, reclassifying an existing position or processing a merit, equity, promotion or other increase. (HR 29 1/2021)

Position Description – System Offices To be completed by supervisors and/or HR Contacts to record the duties, responsibilities, and qualifications of all staff positions. Created when a new budgeted position is established and updated for various situations.  (HR 182 9/22)

Doctoral Program Request Form This form is to be completed by the employee who is requesting participating and approval in the Doctoral Program Initiative.

Educational Release Time Program Application and Approval Form This form is used by employees to request release time from work to attend educational classes.

Employee Complaint Form – System Offices To be completed by employee and forwarded to the Executive Director, Employee Services, within seven business days of the action that caused the complaint. (HR 40 05/24)

Requests for a reasonable accommodation under Title I of the American with Disabilities Act (ADA): An employee who would like to request a reasonable accommodation should inform your manager and email Dawn Santo at dsanto@tamus.edu to request required forms, to include the Employee Request form and Medical Certification form from the employee’s healthcare provider.

Sick Leave Pool Form To be completed by employee and employee’s department head for an employee to donate unused sick leave hours to the Sick Leave Pool or to request hours from the pool for his/her own or a family member’s illness or injury. (HR 27 09/22)

Sick Leave Pool/Direct Donation Medical Certification Form for Employee’s Serious Health Condition To be completed by employee’s health care provider for an employee to request hours from the Sick Leave Pool for his/her own serious health condition. (HR 36E 8/17)

Sick Leave Pool/Direct Donation Medical Certification Form for Employee’s Family Member’s Serious Health Condition To be completed by employee and employee’s family member’s health care provider for an employee to request hours from the Sick Leave Pool for a family member’s serious health condition. Sick Leave Direct Donation – Donor Form To be used by employees to donate unused sick leave hours to another employee. (HR 1771-D 1/16)

Sick Leave Direct Donation – Recipient Form To be used by employees to accept a direct donation of unused sick leave hours from another employee. (HR 1771-R 1/16)

Sick Leave Donation Tax Withholding Form To be used by administrators to submit information about a non-salary compensation item that an employee has received as a part of the sick leave donation process. (HR 1771-T 1/16)

Out-Processing Checklist – System Offices To be completed by a terminating/retiring employee’s departmental processor on or before the employee’s last day of employment to ensure that all of the required out-processing steps are completed. (HR 34 4/24)

Student Out-Processing Checklist – System Offices To be completed by a terminating student employee’s departmental processor on or before the student employee’s last day of employment to ensure that all of the required out-processing steps are completed. (HR 34-student 4/24)

Unpaid Intern & Volunteer

Unpaid Student Intern/Volunteer Waiver To be completed by unpaid student interns volunteering for work and/or individuals who are donating hours of service for civic, charitable or humanitarian reasons. Must be completed prior to the donation of time and services. (HR 206 5/22)
