- Background Check Authorization Must be completed by the finalist for a position before a formal offer of employment is extended. (HR 32 5/09)
- Minor's Employment Release Must be completed for employees younger than age 18 and must be retained for duration of employment. (HR 200 10/01)
- Statement of Selective Service Registration Status Must be completed by male finalists for positions before a formal offer of employment is extended. (HR 203 10/01)

- Request for Personnel Action (Microsoft Word) To be completed by the department head if creating a new position, reclassifying an existing position or processing a merit, equity, promotion or other increase. (HR 29 9/09)
During Employment
- Change of Address A change in residence address, mailing address, phone number (and other personal data) may be updated by the employee through HRConnect while in active status or by completing this form. (HR 183 11/01)
- Complaint Form To be completed by employee and forwarded to the Director, Human Resources, within seven business days of the action that caused the complaint. (HR 40 7/07)
- Direct Deposit Authorization To be completed by employee to enroll, change or cancel direct deposit information. Must be accompanied by a voided check or deposit slip or reflect the electronic routing number and account number. (HR 31 7/09)
- Employee Personal Data To be completed if a change to personal data is needed. Most of this information may now be updated by the employee through HRConnect. (HR 181 11/09)
- External Employment and Consulting Application and Approval To be completed by employees who hold employment outside the A&M System. (HR 202A 7/05)

- Intern/Volunteer Waiver To be completed by unpaid student interns volunteering for work. (HR 205 10/01)
- Position Description (NOTE: Please save this as a 2003 Word document. Saving as 2007 will cause formatting issues.) To be completed by supervisors and/or employees. Used to record the duties, responsibilities, qualifications sought and fiscal impact of classified and non-classified staff positions. Created when a new budgeted position is established and updated for various situations. (HR 182 3/06)
- Prevention of Alcohol Abuse and Illicit Drug Use brochure Brochure about A&M System policy and regulations, legal sanctions and health risks related to alcohol abuse and illicit drug use.
- Returning from Military Leave Application for Re-employment To be completed to apply for re-employment within 90 days after discharge. (HR 37 2/06)
- Sick Leave Pool Form To be completed by employees who want to donate unused sick leave hours to the Sick Leave Pool or who want to request a withdrawal of hours from the pool for illness or injury. (HR 27 1/05)
- Sick Leave Pool Medical Certification Form for Employee's Serious Health Condition To be completed by employees to request a withdrawal of hours from the Sick Leave Pool for his/her own serious health condition. (HR 36E 7/09)
- Sick Leave Pool Medical Certification Form for Employee's Family Member's Serious Health Condition To be completed by employees to request a withdrawal of hours from the Sick Leave Pool for a family member's serious health condition. (HR 36F 7/09)
- Volunteer Waiver To be completed by individuals other than student interns who donate hours of service for civic, charitable or humanitarian reasons. Must be completed before the individual donates any time and services. (HR 206 11/08)
